Sage Leaf Wellness offers Mental Health Therapy services including diagnostic assessment, individual, couples, and family therapy. Extended sessions can be requested as needed.

HOW IT WORKS
In-Network
If you plan to use your in network benefits, claims will be submitted directly to your insurance company. Claims are submitted at a higher rate and reduced per the insurance plans contract with SLW. Once this claim is processed according to your benefits, your financial responsibility will be indicated through generated invoices/statements from Sage Leaf Wellness. These details can be accessed through the client portal once you are an established client. You will also receive an Explanation of Benefits (EOB) from your insurance company outlining your benefits and financial responsibility. All copays are due at the time of service.
Out-of-Network
If you hope to use your insurance benefits, yet you don’t see your particular insurance plan on the list, we will kindly submit a courtesy bill on your behalf. In this event, you, the client are responsible for full payment at the self-pay rate, at the time of service. A courtesy bill is then submitted on your behalf. This prompts your insurance company to apply the amount paid to your out-of-network benefits deductible or reimburse you directly according to their fee schedule and your benefits. A Superbill can also be generated for you to submit to your insurance company on your own, if preferred.
Clients with out-of-network benefits are eligible to use FSA or HSA cards to pay for service.
